This form is for you to let us know your views about a placement at St Oswald's, for 4 mornings each week for a year. Please include any additional information that you feel is important. If you need any help completing this, please speak to the SENCO in school.
You now need to provide:
- a prepared statement in support of referral from the parent
- a prepared statement in support of referral from the child
