This form is to apply for payments through the following schemes, depending on your requirements:
- Discretionary Housing Payment Scheme - for help with rent and housing costs
- Discretionary Council Tax Reduction Scheme - for help with council tax
The same form is used whether you are applying for support through one or both of these schemes, and the same level of information is required for all applications.
You should read the guidance notes for both schemes to check whether you are eligible for one or both of the schemes.
Please be aware that our system will 'time out' after 10 minutes spent on each page of this application. We strongly suggest you gather the required information and documentation ahead of completing the form, as it's not possible to save a partially completed form, nor to retrieve information if the time out period is reached.
In particular, you should have prepared a detailed summary of your household income and expenditure, as this information is required for all applications.
You will also need to provide copies of bank statements for all bank accounts you hold showing your income and expenditure over the past 2 months.
Bank statements can be provided as:
