You can use this form you notify us and the Safety Advisory Group that you're holding an event in York.

Before you complete this form, you may need to gather some information and documents first:

• Event Safety or Operations Plan
• Site plan
• General risk assessments
• Fire risk assessment
• Noise risk assessment
• Traffic management plan
• Medical plan
• Copies of relevant Public Liability Insurance

If you're able to supply these documents, you must collect these ready to upload beforehand, as you will only have 10 minutes per page of the form.
