Use this form to invite the Lord Mayor, Sheriff and members of the Civic Party to attend an event in person.

Please consider the Civic Party Event Protocol when preparing for your engagement.

We require 14 days' notice before the date of your event to consider and process invitations. We will endeavour to confirm attendance 5 working days before your event.

To complete this form you'll need to provide:

• your contact details
• your organisation details
• details of the event, including event date, start time and end time, and details of attendees
• details of the event dress code
• details of any formal duties or required speech
