The Strengths and Difficulties Questionnaire (SDQ) is a short screening questionnaire. It has 5 sections that cover details of:

  1. emotional difficulties
  2. conduct problems
  3. hyperactivity or inattention
  4. friendships and peer groups
  5. positive behaviour

For each statement within this form, select the answer:

  • not true
  • somewhat true
  • certainly true

It would help us if you tried to answer each question even if you're not certain.

Give your answers based on the child's behaviour over the last 6 months or this school year.

If you or anyone is concerned that the SDQ identifies potential concerns that are not being addressed by the care plan or the current provision of services, discuss this with the child's social worker as soon as possible.
